2024 FIRST CHOICE 04 Membership Who qualifies to be a dependant of a member? • Spouse or partner of the principal member who is not a member of another scheme - does not include exspouse or ex partner. • Children - biological, adopted, stepchild and foster children provided the appropriate legal documents are submitted with the application. • Blood relative of the first degree of the Principal member - if fully dependent on the principal member for care and financial support. What proof is required by CAMAF of a dependency? • Marriage certificate in the case of a spouse. • Declaration on the application form in the case of a partner - domestic partnership form to be completed. • Birth certificate in the case of your own child. • Legal documents in the case of adoption or foster child. • Sworn affidavit and proof of income and relationship in the case of blood relatives - proof that the blood relative dependants are financially dependent on the member. How do I add a dependant to my membership? To add a dependant, login on the website or mobile app and go to Profile menu and select Beneficiaries or you can download the Additional Dependant form from the website and complete it. If you are an employee of a company, the form and accompanying documentationshould be submitted to the HR department; if you are anindividual you can forward the form and any requiredadditional documentation to CAMAF. With regard to a new born or adopted child you must register the child within 30 days of birth or adoption and contributions will be payable from the first day of the month of birth or adoption; benefits accrue from that date. The same process applies to marriage; contributions are due from the 1st of the month of the marriage and benefits accrue from the same date. What happens in the event of the death of the principal member? CAMAF should be notified immediately and a copy of the death certificate provided. CAMAF will contact the spouse and advise that membership may continue, if required, with the spouse becoming the Principal member. The status of other dependants, registered as such at the time of death of the Principal member will remain unchanged. Where a child or children are orphaned, the eldest child will be entitled to become the Principal member, at the contribution rate of a Principal member, with other child siblings as dependants. When any beneficiary dies, the date of termination would be the last day of the month and contributions would be payable for the full month. Does CAMAF have the right to cancel my membership? Yes - should you or your dependants: • join another scheme. • provide false information or fail to disclose relevant information. • commit fraud. • fail to pay contributions. • allow another person (other than a registered beneficiary) to access benefits using your card. • neglect to inform CAMAF that goods or services paid for by the scheme were not received.
