2024 FIRST CHOICE 18 Where do I find a Designated Service Provider for chronic medication? A list of retail as well as courier pharmacies is available under the Member Login section on our website or mobile app - Chromic menu > Information > Pharmacy Networks. If I am HIV positive where do I obtain my medication? The DSP for this medication is Medipost. What are the advantages of using a courier pharmacy? Your medication is delivered to you at the appropriate time and they will pro-actively apply for registration of your medication. What options do I have if my current pharmacy is not on the network? • Request your pharmacist to consider joining the Network by emailing implementationsdept@ medikredit.co.za for an application form. • Retrieve your prescription from your current pharmacy and choose a Network pharmacy from which to obtain your new supply of medication. • Remain with your current pharmacy and pay the additional costs. How do I register my chronic condition? A Chronic application form, obtainable from the website must be completed by your doctor and submitted to CAMAF, either online or submitted to clinicalrisk@camaf.co.za or faxed to 011 707 8622. Alternatively your doctor or pharmasist can contact CAMAF to register the condition on 0800 200 300. You can also register your condition via the CAMAF mobile app or on the website under the Member Login section. Once your condition has been registered, you will have access to the CONDITION MEDICINE LIST (CML). This is a list of drugs, appropriate for your condition, that do not require authorisation every year. What happens if I am diagnosed with a new chronic condition during the course of the year? If you have been diagnosed with a condition that falls under the PMB CDL or CAMAF's Additional Chronic Diseases then registration of this condition is required before access to chronic medication benefits will be granted - see above. Can my chronic medication be back dated if I realise that my medication qualifies for the chronic benefit? If, prior to your application for authorisation, you did not realise that your medication met the clinical criteria and your claim has been paid from your Acute benefits, CAMAF will re-process those claims up to 120 days back (only in the same benefit year) and pay from your Chronic medication benefit, once you have registered your condition and had approval for your medication. Once approved, must I renew the application for chronic medication each year? Only medication that is not on the CML list (see above) and has to be specially authorised, must be applied for annually.
