5 What proof of a dependency is required? • Marriage certificate in the case of a spouse. • In the case of a Domestic Partner/Live-in Partner, a Domestic Partnership form must be completed. • Birth certificate, if your own child. • Legal documents in the case of adoption or foster child. • Sworn affidavit and proof of income and relationship in the case of blood relatives in the first degree - proof that the blood relative dependants are financially dependent on you. • An affidavit confirming relationship with child dependent/s, if surnames are different. • Proof of study or financial dependency for dependants who are older than 21. How do I add a dependant to my membership (existing members)? ONLINE APPLICATION • Go to Member Login on CAMAF website • Under the Profile menu, select 'Add New Dependant' • Have the following documents available: • Copies of ID's • Marriage certificate, if applicable • Proof of income, if applicable • Certificate of membership with end date, if applicable MANUAL APPLICATION • You may add a dependant by completing an "Additional Dependant" form. Once completed, the form and accompanying documents must be submitted to your HR department. • With regard to a new born or adopted child you must register the child within 30 days of birth or adoption and contributions will be payable from the first day of the month of birth or adoption; benefits accrue from that date (Registration of New Born Baby form must be completed and submitted with a copy of the child's birth certificate). The same process applies to marriage; contributions are due from the 1 st of the month of the marriage and benefits accrue from the same date. 5
