4 What additional documents are required from Vital Plus, Vital Network, Essential Plus, Essential Network, First Choice or Network Choice members? • Proof of income in the form of a salary advice or employment letter. Please note the income means "the gross income, whether in cash or in kind, which accrues from any source whatsoever to the member and his registered dependants." • If proof of income is not received, you will be defaulted to the highest salary category and be charged the monthly contribution for that category. Nominated GPs for network options If you selected Alliance Network, Double Network or Network Choice as your benefit option, it is compulsory that you nominate two GPs for you and each of your beneficiaries from the CAMAF GP Network. You are required to only use these nominated GPs for all GP consultations, referrals and medicine prescriptions. Either of the two nominated GPs can be consulted. You are allowed to change your GP once a year. It is therefore important that you choose your GPs carefully. To find a network GP nearest to you, you can use the Geomapping facility on the CAMAF website. Please email your nominated GP selection for each of your beneficiaries to mygp@camaf.co.za and allow 48 hours for the update to reflect. Ensure that you include the GP practice name and practice number, your name and membership number, as well as your beneficiary name(s). Dependants Who qualifies to be a dependant? • Spouse or partner of the principal member who is not a member of another scheme - does not include divorced or ex-partner. • Children - biological, adopted, stepchild and foster children, provided the appropriate legal documents are submitted with the application. • Blood relative of the first degree of the principal member - if fully dependent on the principal member for care and financial support. What proof of a dependency is required? • Marriage certificate in the case of a spouse. • In the case of a Domestic Partner/Live-in Partner, a Domestic Partnership form must be completed. • Birth certificate, if your own child. • Legal documents in the case of adoption or foster child. • Sworn affidavit and proof of income and relationship in the case of blood relatives - proof that the blood relative dependants are financially dependent on you. • An affidavit confirming relationship with child dependent/s, if surnames are different.
