3 Joining through employer WHAT DOCUMENTS OR INFORMATION IS REQUIRED TO PROCESS MY APPLICATION? Online application • Log onto the CAMAF website • Go to Apply - Online application • Obtain your group name and region number from your HR • You will need to have the following documents available: 1. Copy of ID's/Birth certificates 2. Marriage certificate, if applicable 3. Proof of income, if applicable 4. Certificate of membership with end date Manual application Complete the Employer Group application form (available on the website) • A copy of your and your dependants' identity documents, marriage certificate (where applicable) or birth certificates. • Personal details to be completed in full i.e. address details, contact numbers and email address. • Banking details to pay any refunds due to you. Please note that credit card accounts are not accepted. • The details of any dependant you wish to register. • Employer details i.e. name of company, group region, department, branch, date you started working for the company, date you want to join CAMAF. • Your form must be signed by the principal member and the firm representative, and be stamped with the company stamp. • The medical history section on the form must be completed in full. If you want your medical history to be confidential, please submit this section in a sealed envelope or contact CAMAF as soon as possible after your application has been submitted to advise that you want your medical history to be kept confidential. Confidential details can be emailed to underwriting@camaf.co.za Individual members Please refer to our website, www.camaf.co.za, for the criteria for individual membership. Individual members may also apply Online or complete the Application form for Individual Members, available on the website. PLEASE NOTE: The debit order bank details section must be signed by the account holder whose bank account will be used to debit contributions. If the main member is not the account holder, a consent letter from the account holder and a copy of his/her ID will be required. We will not be able to process your application form unless all of the above requirements are met.
