Your doctor must complete the
Chronic Application Form and email it to

Your doctor or pharmacist contacts CAMAF
on 0800 200 300 to register your condition
Register your condition on the CAMAF mobile app
or on the website, Member Login

You can access your membership card in
a digital format when you log in to the
CAMAF mobile app or Member Login
on the website
The digital card works the same way as your
physical card. You can use it at any service provider
– details on the card will be used to confirm your
CAMAF membership.
Download a copy of your Membership Certificate
from the CAMAF mobile app or from the website,
Member Login under Documents
